Here's What You Need to Qualify:

  • Active California license
  • In-force California Broker's Bond
  • In-force Errors and Omissions policy

    • Get started producing Earthquake business today by completing, printing and faxing all of the items listed below to (866) 830-5432.

    Broker Application Form
    Broker's Agreement
    W-9 Form
    EDD Form  (If applicable)

    California Fire and Casualty Agent/Broker license

    California Broker's Bond

    Errors and Omissions Declaration Page


    We'll quickly process your paperwork and notify you when your application has been approved. You'll receive a fully executed copy of our Broker's Agreement and we'll set you up to access our on-line earthquake policy system. We will provide complete training on our products to your staff, as needed.
